 Family
Support Services - UCP provides family
support services to individuals with developmental disabilities of all
ages through grant funded case management services. The purpose of these
services is to enable the individual to remain in the community, facilitate
independence and maximize their full potential. The intensity and type
of services provided is determined by the individual's needs and desire
for assistance. Services may include home visits, assessments, adaptive
toy and equipment loan, individual program plans, referral and advocacy
for other services, and disability related workshops.
Referrals
may include linkage to therapies, respite, in-home supports, medical clinics,
and special education advocacy groups. UCP may also assist with locating
and referring to residential providers. It is our policy to offer in-home
support when appropriate to enable children to remain with their families
as long as possible. All services are of no cost to the family.
Early Intervention Services - . Early Intervention
services are state funded services for children 0-3 years of age who have
developmental delays. UCP provides developmental therapy and parent liaison
services funded by this system. These services may include home visits,
assessment of your child's strengths and needs, development of a individualized
family service plan to help your child meet his full potential, loan of
adaptive toys and equipment that are appropriate for your child's development,
consultation with therapist's and doctors, therapeutic swim program and
referral to other support services.
Children receiving EI services are also eligible for
services covered under family support services and transitioning case
management for children 2.5-4 years of age. All services are of no cost
to the family.

Transitioning Case management
- UCP provides transitioning support for children and adolescents with
developmental disabilities (Macon County only). These services are funded
by our Macon County Mental Health Board and are of no cost to the individual
or his family. These services include individual consultation with case
managers and disability related workshops for the family
For Children 2.5-4 years of age
- Transitioning services include assisting the family with the transition
from the early intervention system into the special education system or
other appropriate services. Transitioning case managers will provide information
regarding special education rights and options for services when Early
Intervention services discontinue at age 3. Some children may be eligible
for special education services including early childhood education, speech,
physical or occupational therapy, vision resource services or other supports
needed for the child to be included in the least restrictive education
environment. Families may choose alternative settings for their child
such as day care other preschool settings and therapies outside the special
education system or a combination of various services.
For Adolescents 13-21 years of age
- Transitioning services include assisting the adolescent and his family
with the transition from school into the community. This may include vocational,
post secondary education, residential or other options in the community.
The case manager will provide information about the individual's disability,
legal rights, legal matters regarding guardianship and emancipation and
other relevant issues that will assist the family in the transition process.
Many families are interested in further day programming and possible residential
programs after graduation. UCP case managers are knowledgeable in this
area and can assist in referrals and arranging tours of these facilities.
